Getting started as a patient

If this is your first visit to Aperta Health, you'll need to create a free account before booking.

1. 1Enter your legal first and last name
2. 2Provide your email address — this is your login
3. 3Add your date of birth and phone number
4. 4Select your home location (Idaho Falls or Middleton)
5. 5Create a secure password
6. 6Click Create Account

Your patient portal is where you manage everything — appointments, consent forms, membership, and your profile.

1. 1Enter the email address you registered with
2. 2Enter your password
3. 3Click Sign In
4. 4You'll land on your personal dashboard

Note: Forgot your password? Use the "Forgot password" link on the login page to reset it via email.

Before your first visit, you'll need to sign one combined consent form covering chiropractic care, HIPAA privacy, and our financial policy.

1. 1From your dashboard, click Consent Forms in the left menu
2. 2Click Sign Now on the Patient Consent & Policy Agreement
3. 3Read through all three sections: informed consent, HIPAA acknowledgment, and financial policy
4. 4Type your full legal name as your electronic signature
5. 5If signing for a minor, check the guardian box and follow the prompts
6. 6Click Sign & Submit — you're done

Note: Your signed consent is saved permanently to your account. You can view it any time from the Consent Forms page.

Your health intake lets your provider know about your current health, goals, and any conditions or medications — so your first visit runs smoothly.

1. 1From your dashboard, click Health Intake in the left menu
2. 2Answer questions about your current complaint and health history
3. 3List any current medications
4. 4Describe your health goals
5. 5Click Submit Intake when complete

Note: You can update your intake at any time if your health situation changes.

You can book appointments any time — day or night — directly from your portal.

1. 1From your dashboard, click Book a Visit in the left menu
2. 2Select your preferred location (Idaho Falls or Middleton)
3. 3Pick a service (e.g. Chiropractic Adjustment) — member and non-member prices are shown
4. 4Choose an available date — colored dots on the calendar show which providers have openings
5. 5Pick a time slot and review your appointment details
6. 6Click Confirm Booking
7. 7You'll receive a confirmation email to the address on your account

Note: Reminder texts are sent automatically before your appointment. Reply Y to the reminder to confirm — your provider will see it on the schedule.

An Aperta Health membership reduces your per-visit cost from $49 to $22. If you plan on coming regularly, it pays for itself in just two visits a month.

2. 2Choose your plan: Individual ($19/mo), Partner Plan ($19/mo base + $10/mo per partner), or Family ($39/mo)
3. 3Click Subscribe and enter your payment details
4. 4Your member rate is active immediately and honored at all Chiro Shack locations

Note: Membership is month-to-month with no long-term commitment. You can cancel any time from the Membership page.
